Un faux asthme lié à une compression bronchique par une artère pulmonaire gauche à trajet aberrant chez une enfant de 7 ans - 17/04/08

Introduction

Devant les sibilants d'un enfant le diagnostic d'asthme est très probable, cependant l'absence de réversibilité aux bêta-2-mimétiques doit faire évoquer d'autres étiologies.

Cas clinique

Les auteurs rapportent le cas d'un enfant de 7 ans adressé en consultation pour bilan d'asthme difficile évoluant depuis plusieurs années. L'absence de réversibilité aux bêta-2-mimétiques, les signes indirects de distension thoracique aux explorations fonctionnelles respiratoires, la présence d'un débord cardiaque droit ont refait discuter le diagnostic. L'angioscanner a révélé une anomalie de l'artère pulmonaire. L'angiographie pulmonaire a confirmé le diagnostic d'artère pulmonaire gauche à trajet aberrant. Cette malformation vasculaire a été responsable d'une compression bronchique extrinsèque intermittente. Le traitement a été chirurgical permettant la disparition de la symptomatologie.

Conclusion

Nous rapportons ce cas en raison du caractère exceptionnel du mode de découverte de cette anomalie vasculaire : à l'âge de 7 ans, après 2 ans d'évolution d'une symptomatologie évocatrice d'asthme.

Bronchial obstruction by an aberrant left pulmonary artery misdiagnosed as asthma in a 7 year old child

Introduction

Wheezing is common symptom in infants and is usually due to asthma. However an alternative diagnosis should be sought if there is no reversibility to B2-agonist.

Case report

This case report describes a 7 years old child who had been treated for poorly controlled asthma for several years. The absence of B2-agonist reversibility, indirect signs of thoracic straining on spirometry and evidence of right heart decompensation raised doubts about the diagnosis. CT angiography demonstrated a pulmonary artery malformation. Formal pulmonary angiography confirmed the diagnosis of pulmonary artery sling. This malformation had been causing intermittent bronchial compression and the symptoms resolved after surgical intervention.

Conclusion

Wheezing symptoms over two years in a child, misdiagnosed as asthma, is an unusual presentation of this pulmonary vascular anomaly.
