Résumé

L’évolution métastatique cérébrale est fréquente dans les cancers bronchiques et la médiane de survie est alors limitée. Le traitement standard pour les patients ayant plusieurs métastases cérébrales est l’irradiation de l’encéphale en totalité. Cependant, même si dans la plupart des cas on observe une amélioration des signes neurologiques, le contrôle local reste faible (50%). Le but de la radiothérapie cérébrale en condition stéréotaxique est d’augmenter de manière très localisée au niveau du volume-cible la dose d’irradiation, sans augmenter la dose aux organes avoisinant. Le but in fine est d’augmenter le contrôle local, possiblement la survie des patients sans augmenter la toxicité neurologique à long terme. Il existe 2 types de radiothérapie cérébrale en conditions stéréotaxiques : la radiochirurgie et la radiothérapie stéréotaxique hypofractionnée. Ces 2 techniques permettent une augmentation du contrôle local jusqu’à environ 85%.

Summary

Brain metastasis is the most common central nervous system neoplasm, very frequently affecting patients with lung cancer. Therefore, survival of these patients is limited.

Traditionally, whole brain radiation therapy has been the accepted standard treatment for most patients with brain metastases and has contributed to reduce neurological symptoms. However, local control is still low (50%).The recent introduction of stereotactic radiotherapy has changed the treatment paradigm and improved the outcome of metatastic brain tumors with a better local control by increasing the dose of radiation therapy in limited metastases without increasing the dose to the neighbouring organs. There are 2 types of stereotactic radiation therapy: radiosurgey with only one fraction of delivered radiation and hypofractionnated stereotactic radiation therapy with a low number of fractions. These techniques enable an increase in the local control of up to approximately 85%.
