Résumé

Le traitement standard des patients présentant un carcinome bronchique non à petites cellules (CBNPC) localement avancé inopérable, repose actuellement sur une radiothérapie associée à une chimiothérapie à base de sels de platine, délivrées de façon concomitante chez les patients en bon état général. La méta-analyse sur données individuelles ayant inclus des études randomisées comparant une CTRT séquentielle à une CTRT concomitante a montré un bénéfice en faveur de la CTRT concomitante avec une survie à 5 ans de 15%. La survie médiane peut dépasser 24 mois dans des études de phase II et III de CTRT récemment publiées comportant une radiothérapie conformationelle. L’implémentation de nouvelles techniques de radiothérapie conformationnelle, et l’intégration de nouveaux traitements systémiques, certains ciblés en fonction du profil moléculaire des patients constituent des nouveaux axes de recherche. Il est important de continuer à inclure les patients dans des études prospectives afin d’améliorer leur devenir.

Summary

In locally advanced inoperable non-small cell lung cancer (NSCLC) patients, combined radiotherapy and platinum-based chemotherapy, administered concomitantly in good performance status patients, is the standard treatment. Based on a meta-analysis of randomized studies comparing sequential chemoradiotherapy (CTRT) to concomitant CTRT, there was a benefit in favour of concurrent CTRT with a 5-year survival of 15%. In recent phase II, III studies where more conformal RT was used, reported median survival was over 24 months. The implementation of new techniques of optimized conformal radiotherapy and the integration of new systemic therapies, as well as targeted agents according to the molecular profile of patients constitute some of the areas of research for the future. Inclusion of patients in prospective studies is warranted to improve the outcome of locally advanced NSCLC.
