Traitement du cancer bronchique non à petites cellules métastatique chez les patients âgés: vers une évolution des pratiques - 16/12/11

Trop souvent, les sujets âgés n’ont pas la place qu’ils méritent dans les essais cliniques. Bien qu’ils constituent presque un tiers des patients atteints de CBNPC, nous manquons de données objectives sur les spécificités de cette population, ce qui conduit souvent à une prise en charge sous optimale.
Les recommandations de l’ASCO 2004 et de l’EORTC en 2010 sont en faveur d’une monochimiothérapie par une drogue de 3e génération. Elles se basent sur un essai de 1999 qui avait comparé la vinorelbine aux soins de support seuls. D’autres essais évaluant des associations sans sels de platine s’étaient révélés non concluants.
Un essai de phase III mené par l’IFCT et publié cette année a comparé l’association de carboplatine et paclitaxel hebdomadaire à une monochimiothérapie par gemcitabine ou vinorelbine. La médiane de survie passe de 6,2 à 10,3 mois et le taux de survie à un an de 25,4 à 44,5 % chez une population peu sélectionnée avec un PS à 2 chez 27 % des malades et une médiane d’âge 77 ans. Ces résultats devraient faire évoluer rapidement les recommandations pour le traitement des sujets âgés. De nombreuses questions se posent encore rendant souhaitable la réalisation d’autres essais spécifiques pour cette population.

Elderly are often under-represented in clinicals trials. Despite the fact that they represent nearly a third of the patients with non small cell lung cancer there is a lack of specific data on this subgroup and they may therefore not receive an appropriate treatment.
The ASCO 2004 and EORTC 2010 guidelines recommended monochemotherapy with a third generation drug. It is based on a trial conducted in 1999 which compared Vinorelbine with best supportive care. Other trials looking at combinations without platinum were unconclusive.
A phase III study from the IFCT and published this year compared the association of carboplatine and weekly paclitaxel with a monochemotherapy of gemcitabine or vinorelbine. Median overall survival was 10.3 months for doublet chemotherapy and 6.2 months for monotherapy and one-year survival was 44.5% and 25.4% respectively in a quite unselected group with PS 2 in 27% of patients and a median age of 77. These results should prompt a quick change in the guidelines for the treatment of the elderly and new studies dedicated to old patients are needed.
