Résumé

Le standard de la chimiothérapie de 1ère ligne des CBNPC associe un sel de platine et un cytotoxique de 3e génération, avec une efficacité similaire des différents schémas thérapeutiques. Les progrès ont été limités aux carcinomes non-épidermoïdes avec la stratégie de maintenance utilisant la poursuite du pemetrexed en monothérapie et l’adjonction du bevacizumab. Le développement en 1ère ligne de l’immunothérapie avec les anti-PD-1 et anti-PD-LI transforme l’algorithme de traitement des CBNPC avancés. Comparativement à la chimiothérapie de 1ère ligne, le pembrolizumab améliore la survie dans les CBNPC exprimant PD-L1 dans plus de 50 % des cellules tumorales. Les associations combinant chimiothérapie et anti-PD-1/PD-L1 s’avèrent supérieures à la chimiothérapie seule et devraient être principalement indiquées dans les tumeurs exprimant peu ou pas PD-L1, conservant la nécessité d’une chimiothérapie de 1ère ligne pour la majorité des patients.

Abstract

First-line treatment of metastatic NSCLC without oncogenic addiction remains based on chemotherapy regimen combining a platinum salt and a third generation cytotoxic drug, with similar results for the different available schedules. The improvement of chemotherapy results has been limited to non-squamous carcinoma, deriving from maintenance strategies mainly based on continuation of pemetrexed as a single agent and on addition of bevacizumab to chemotherapy. The development of immunotherapy in first-line therapy with anti-PD-1 and anti-PD-LI is changing the first line treatment algorithm of advanced NSCLC. The anti-PD-1 pembrolizumab clearly improves the overall survival in NSCLC with PD-L1 expression in greater than 50% of tumour cells, comparatively to cytotoxic chemotherapy. Combinations of anti-PD-1/PD-L1 to platinum-based chemotherapy are superior to chemotherapy alone and should be mainly indicated for NSCLC with no or low PD-L1 expression; therefore, cytotoxic chemotherapy would remain needed for the majority of advanced NSCLC patients.
