Résumé

Introduction

L’épidémiologie du cancer bronchique évolue et met les soignants en présence d’un profil émergeant : des femmes, parfois jeunes. L’exploration des effets de ce changement en cours auprès d’une population soignante (infirmières et aide-soignantes) en services d’oncologie thoracique n’a pas fait l’objet d’évaluation.

Méthode

Une étude quantitative permet de maîtriser les biais pouvant avoir un impact sur l’identification aux patientes. Une étude qualitative catégorielle et automatisée (logiciel Tropes) de productions de discours relevant de « situations difficiles » est réalisée.

Résultats

Quatre contextes activateurs de situations critiques sont identifiés : désaccord et conflit dans la prise en charge, situation de prise en charge contrainte, situation de débordement notamment liée à une souffrance accrue du patient et ressenti d’isolement dans la prise en charge. Concernant la prise en charge des patientes, après l’impact des désaccords, le ressenti de prise en charge contrainte en présence d’une patiente à laquelle les soignantes s’identifient est plus prégnant.

Conclusion

Un accompagnement en équipe des soignantes est à privilégier mais il n’est pas suffisant. Seul un contexte de formation centré sur la pratique et mettant en présence de pairs n’appartenant pas à l’équipe paraît permettre une élaboration des situations les plus impactantes.

Summary

Introduction

The epidemiology of lung cancer is evolving and caregivers need to address an emerging demographic, women, sometimes presenting at a young age. The effect of this ongoing change on thoracic oncology services and on the nursing population (registered nurses and auxiliary nurses) has not been evaluated.

Methods

A quantitative analysis can help to control the biases that may have an impact on nursing staff identification with female patients. A categorical and automated qualitative study (Tropes software) of speech productions related to “difficult situations” was carried out.

Results

Four contexts activating critical situations were identified: disagreement and conflict in care, a constraining situation of care, an overflow situation, particularly related to increased suffering of the patient and a feeling of being isolated in care delivery. Regarding the management of female patients, after the impact of disagreements in management, the feeling of care being constrained in the presence of a female patient with whom caregivers identify themselves is more significant than with regard to male patients.

Conclusion

Support within the care team is essential, but not sufficient. Only a practice-based training context involving peers outside the team seems to help address the most impactful situations.
