Introduction

L'objectif de cette étude prospective ouverte, non contrôlée, est d'apprécier la faisabilité et l'efficacité de 10 semaines de réentraînement à l'effort à domicile sur ergocycle chez des malades respiratoires.

Methodes

75 patients sont adressés consécutivement. 37 (75 % BPCO) sont inclus, 38 (51 %) sont exclus. Neuf abandonnent, cinq ont des données manquantes : l'analyse porte sur 23 patients.

Resultats

Nous observons une amélioration significative de la qualité de vie (CRQ), p = 0,02, de l'endurance, p ≪ 0,001, de la puissance de travail sur ergocycle, p ≪ 0,001. Il existe une corrélation entre l'augmentation de la puissance de travail et amélioration de la qualité de vie (p = 0,01, r = 0,5). La répartition des patients en fonction de l'amélioration de la puissance de travail (faible (0 à 10 %) – modérée (11 à 49 %) – forte (> 50 %)) montre qu'une faible amélioration est retrouvée chez ceux dont la puissance maximale initiale est basse.

Conclusion

Notre expérience conforte l'efficacité du réentraînement à domicile. La faisabilité concerne un patient sur deux. Les co-morbidités, le contexte psychosocial sont les principaux facteurs limitant la mise en place et l'observance. L'amélioration de la capacité d'effort semble être un déterminant important de la qualité de vie. L'ergocycle, pourrait exiger un effort trop important pour le réentraînement des patients très sévères.

Home pulmonary rehabilitation: results in a cohort of 37 patients with respiratory handicap

Introduction

The purpose of this open, prospective, non-controlled study is to evaluate the feasibility and effect of 10 weeks of home-based exercise retraining on a cyclo-ergometer in respiratory patients.

Methods

75 patients are consecutively referred. 37 patients, (75% COPD), took part in the programme, 38 (51%) did not. Nine dropped out, five had missing data, therefore the analysis is of 23 patients.

Results

There was a significant improvement in Quality of life (CRQ) (p = 0,02), in endurance and in work rate on the bicycle (p ≪ 0.001). There is a correlation between the increase in work rate on the bicycle and the improvement in quality of life (p = 0.01, r = 0.5). When the patients are divided according to the level of improvement in work rate on the cycle-ergometer (mild 0 to 10% improvement; moderate 11 to 49% and strong > 50%) we find that only mild improvement is found in those with a low initial work rate.

Conclusion

We confirm the effectiveness of home based training, this being feasible in one out of two patients referred. Co-morbidity and the psycho-social situation were the main exclusion factors. The improvement in effort capacity could be an important determinant of quality of life. Cyclo-ergometer could required too great an effort for training more severe patients.
