Survenue précoce d’un carcinome bronchique à petites cellules chez un sujet jeune au Bénin - 08/09/22

La survenue d’un cancer bronchique chez les sujets très jeunes (<30 ans) est rare. Il survient souvent après la quarantaine et surtout dans un contexte de forte intoxication tabagique. Nous rapportons un cas clinique illustrant la survenue précoce de carcinome bronchique à petites cellules chez le jeune.

Il s’agit d’un homme de 30 ans, tabagique actif depuis 10 ans environ (15 paquets années) qui a consulté pour une toux avec crachats hémoptoïques. Les examens cliniques et paracliniques ont permis de diagnostiquer un carcinome bronchique à petites cellules à droite avec quelques nodules métastatiques controlatéraux. Il fut classé au stade T2bN2M1a. L’évolution a été rapidement fatale avec décès au bout d’un mois, faute de ressources financières pour bénéficier d’une chimiothérapie anticancéreuse adaptée.

La précocité du carcinome bronchique chez le jeune tabagique, plaide pour un renforcement de la lutte contre le tabac chez les jeunes adolescents en général, surtout en Afrique où le phénomène prend de l’ampleur.

Context Bronchial cancer in a person under 30 years of age is quite rare. It generally occurs after 40 years of age following heavy smoking intoxication. We report a clinical case illustrating the early onset of a small cell lung carcinoma in young heavy smoker.

A 30-year-old patient, current smoker for about 10 years (15 packs/year), consulted for a cough with haemoptotic sputum. Clinical and paraclinical examinations diagnosed small cell carcinoma of the right lung with some controlatéral metastatic nodules. _. He was classified as stage T2bN2M1a. Unfortunately, due to lack of financial accessibility to suitable chemotherapy, the patient died after one month.

Early-onset of bronchial carcinoma in young smokers calls for strengthened control of teenage tobacco use, especially in Africa, where the phenomenon has been taking on alarming proportions.
