Introduction Les accidents thermiques se compliquent dans 25 % des cas de manifestations respiratoires, répondant à des mécanismes lésionnels multiples. En phase aiguë, l’œdème des voies aériennes, le syndrome d’inhalation de fumées, le SDRA en sont les principales étiologies, engageant le pronostic vital dans 60 % des cas. Les complications respiratoires tardives sont méconnues ou négligées. Il s’agit de troubles ventilatoires obstructifs, succédant au syndrome d’inhalation, et restrictifs secondaires au SDRA ou à des séquelles cutanées.

Observation Nous rapportons le cas d’une patiente, brûlée 2 ans auparavant, hospitalisée pour décompensation respiratoire aiguë grave compliquant une BPCO post-tabagique. La découverte d’un trouble ventilatoire mixte sévère a permis d’évoquer la responsabilité d’étiologies alternatives, parmi lesquelles les conséquences d’un SDRA et de cicatrices thoraco-abdominales diffuses, circulaires. Ces dernières réalisant un « blindage » thoracique complet semblent de bonnes candidates pour expliquer le trouble ventilatoire restrictif.

Conclusions Ces constatations fonctionnelles respiratoires et la gravité potentielle des décompensations respiratoires aiguës plaident pour un suivi pneumologique régulier des brûlés avec lésions cicatricielles thoraciques circulaires et la nécessité de les considérer comme à haut risque de complications respiratoires.

Late respiratory function complications following burns

Introduction Twenty five per cent of thermal injuries are associated with secondary respiratory events linked to several mechanisms. In the acute phase of the accident oedema of the airways, the fume inhalation syndrome and ARDS are the most common causes responsible for death in 60% of cases. Late respiratory complications are little known and neglected. They comprise obstructive ventilatory defects due to the inhalation syndrome and restrictive defects secondary to ARDS or to dermal injury.

Case report We report the case of a female patient, extensively burnt 2 years previously, admitted to hospital with severe acute respiratory failure complicating COPD. The presence of both restrictive and obstructive defects led to the suggestion of alternative underlying mechanisms such as the pulmonary consequences of ARDS and extensive dermal scars. The latter were responsible for an armour like thickening of the skin of the thorax compatible with the restrictive defect.

Conclusions These functional abnormalities and the potential severity of acute respiratory failure are indications for regular pulmonary follow-up of patients with severe circumferential scarring of the thorax who are at high risk for respiratory complications.
