Résumé

Introduction

L’érythème pigmenté fixe (EPF) est pathognomonique d’une toxidermie et représente la seule dermatose de cause exclusivement médicamenteuse. Les antibiotiques antituberculeux sont parmi les médicaments habituellement responsables, parmi lesquels la rifampicine, moins souvent l’isoniazide et le pyrazinamide. L’EPF secondaire à la prise d’éthambutol a rarement été décrit.

Observation

Un patient de 32ans, VIH négatif, consulte pour un EPF localisé à la face interne des lèvres et aux plis interdigitaux, au 4e mois d’un traitement antituberculeux comportant la rifampicine, l’isoniazide et l’éthambutol. Le diagnostic est porté devant l’aspect caractéristique de la lésion d’EPF et la réapparition précoce des lésions récidivant exactement aux mêmes endroits, après la réintroduction accidentelle de la trithérapie antituberculeuse à base d’éthambutol et la tolérance parfaite de la bithérapie rifampicine et isoniazide.

Conclusion

La découverte d’un EPF impose une démarche étiologique rigoureuse à la recherche du médicament responsable. Au cours du traitement antituberculeux, le praticien doit avoir à l’esprit le rôle potentiel de l’éthambutol, dont le rôle est possiblement potentialisé par la rifampicine.

Summary

Introduction

Fixed drug eruption (FDE) is a specific skin reaction and the only exclusively medicinal dermatosis. Among the drugs usually responsible are the antituberculous antibiotics including rifampicin and, less often, isoniazid and pyrazinamide. FDE after taking ethambutol is rarely described.

Case report

A 32-year old HIV negative patient presented a FDE localized to the internal surface of the lips and the interdigital folds during the 4th month of antituberculous treatment comprising rifampicin, isoniazid and ethambutol. The diagnosis was supported by the characteristic appearances of the lesions of FDE and their early reappearance in the same areas after accidental reintroduction of antituberculous triple therapy including ethambutol. Double-agent therapy with rifamicin and isoniazid was tolerated well.

Conclusion

Discovery of FDE requires a rigorous search for the responsible medicine. During antituberculous treatment, the practitioner has to bear in mind the potential role of ethambutol, which is possibly potentiated by rifampicin.
