Chimiothérapie périopératoire du cancer bronchique non à petites cellules (CBNPC) vue par le chirurgien - 09/01/16

Introduction |
La chirurgie constitue le traitement de choix des CBNPC de stades précoces. Malgré une chirurgie à visée curative, il existe un risque non nul de récidive et de décès par cancer.
État des connaissances |
La chimiothérapie adjuvante est indiquée pour les stades II et III, débattue dans les stades IB. Les méta-analyses publiées ont conclu à un bénéfice de survie absolu de 5 % à 5 ans. La place de la chimiothérapie néoadjuvante est moins codifiée. La seule méta-analyse publiée comparant les deux schémas n’a pas noté de différence significative en termes de survie.
Perspectives : Le bénéfice de l’utilisation des inhibiteurs de tyrosine kinase (ITK), chez les malades mutés Epidermal Growth Factor Receptor (EGFR) de stades avancés, a fait se poser la question de l’intérât de leur utilisation périopératoire chez les stades précoces. À ce jour, peu de données sont disponibles dans la littérature, mais sont encourageantes quant à leur intérât en situation néoadjuvante afin d’augmenter la résécabilité et, en adjuvant afin d’allonger la survie sans progression, chez des malades sélectionnés.
Conclusion |
Le moment de la chimiothérapie semble avoir peu d’importance sur la survie. La place des ITK, bien que prometteuse, reste à définir.

Introduction |
Up to now, surgery remains the treatment of choice for early-stage nonsmall cell lung cancer. However, despite surgery with curative intent, risk of recurrence and cancer-related deaths are major concern.
State of the art |
Use of adjuvant chemotherapy is supported by evidences in stages II and III, however more controversial in stage IB. Published meta-analyses have reached conclusion that adjuvant chemotherapy leads to an absolute 5-year overall survival benefit of 5%. The single published meta-analysis comparing adjuvant and neo-adjuvant chemotherapy did not observe a significant difference on overall survival according to the two strategies.
Perspectives: The dramatic benefit of tyrosine kinase inhibitors in advanced-stage nonsmall cell lung cancer harboring the EGFR mutation have led to question whether they might be interesting in the peri-operative setting for early stages. So far, few data are published, but seem very interesting regarding their interest as neo-adjuvant regimen in order to increase resecability, and in adjuvant settings to increase disease-free survival, in selected patients.
Conclusion |
Regarding the published evidence, chemotherapy timing does not seem to impact overall survival. Role of tyrosine kinase inhibitors in peri-operative setting, even very interesting, has to be clearly defined.
