Résumé

Introduction

Les kystes cœlomiques intrathoraciques sont des tumeurs bénignes très rares d’origine embryonnaire tapissées par un mésothélium. Ils sont le plus souvent asymptomatiques, découverts de façon fortuite sur une radiographie thoracique et sont principalement localisés dans l’angle cardiophrénique réalisant les kystes pleuropéricardiques.

Observation

Un homme âgé de 50ans a été adressé pour des douleurs thoraciques gauches atypiques depuis deux ans. L’examen physique et la radiographie thoracique étaient sans particularité. La tomodensitométrie thoracique a montré la présence d’un kyste du médiastin supérieur et postérieur au niveau de la gouttière costovertébrale gauche. Compte tenu de sa localisation atypique et de son caractère symptomatique, une kystectomie par thoracotomie postérolatérale a été réalisée permettant de poser le diagnostic de kyste cœlomique ectopique. Le patient rapporte une disparition des douleurs thoraciques après la résection chirurgicale, avec un recul de 3ans. Par ailleurs, le scanner thoracique a visualisé une artère sous-clavière rétro-œsophagienne qui est une anomalie des arcs aortiques d’origine congénitale de découverte fortuite. À notre connaissance, l’association de ces deux anomalies congénitales n’a jamais été rapportée auparavant dans la littérature.

Conclusion

La localisation ectopique du kyste cœlomique dans le médiastin postérieur et supérieur au niveau de la gouttière costovertébrale gauche est très rare, imposant sa résection chirurgicale à la fois à visée diagnostique et thérapeutique d’autant plus qu’il est symptomatique.

Summary

Background

Intrathoracic mesothelial cysts are congenital lesions due to an abnormal development of the pericardial coelom. They are usually asymptomatic and found incidentally on chest radiography or computed tomography. As their classic anatomical location is in the cardiophrenic angle, they are also referred to pleuropericardial cysts.

Case report

A 50-year-old male presented with a history of chest pain. Physical examination and chest X-ray were normal. Computed tomography (CT) scan revealed a cystic lesion in the posterior and upper mediastinum. The cyst was surgically removed through a posterolateral thoracotomy. Histopathological examination confirmed that it was a mesothelial cyst. The surgical resection of the cyst lead to relief of the thoracic pain over a three-year follow-up period. CT-scan showed an aberrant right subclavian artery or arteria lusoria, which is an anomaly of the aortic arch secondary to abnormal embryogenesis. We know no other report of concurrent ectopic coelomic cyst and aberrant right subclavian artery.

Conclusion

Although the majority of coelomic cysts needs only radiological and clinical follow-up, surgical resection should be performed when the patient is symptomatic or when the diagnosis is uncertain.
