Résumé

L’immunothérapie ciblant les points de contrôle de la réponse immunitaire est devenue un standard de traitement pour les cancers bronchiques avancés et métastatiques, en modifiant les algorithmes de prise en charge dès la première ligne thérapeutique. L’immunothérapie a également été évaluée en situation de tumeurs intra-thoraciques rares, avec des problématiques et des enjeux spécifiques. En situation de mésothéliome malin pleural, l’immunothérapie a été évaluée principalement en lignes avancées, avec des agents ciblant PD-1 ; l’essai CheckMate-743 démontre en première ligne l’intérêt de l’association nivolumab et ipilimumab. En situation de tumeur thymique, l’immunothérapie est généralement contre-indiquée dans les thymomes et ne constitue pas une option thérapeutique standard pour les carcinomes thymiques. L’inclusion de patients dans les essais cliniques en cours est ainsi le principal message pour notre pratique clinique.

Abstract

Immunotherapy targeting immune checkpoints has become a standard of treatment for advanced and metastatic lung cancers leading to a major update of treatment algorithms in the first-line setting. Immunotherapy has also been evaluated in rare intrathoracic tumors, with specific issues and challenges. In malignant pleural mesothelioma, immunotherapy has been evaluated mainly in advanced lines, with agents targeting PD-1 ; the CheckMate-743 trial demonstrates the benefit of the combination of nivolumab and ipilimumab as a first-line treatment. In thymic tumors, immunotherapy is generally contraindicated in thymomas and is not a standard therapeutic option for thymic carcinomas. The enrollment of patients in ongoing clinical trials is therefore the main message for clinical practice.
