Outcome of 248 patients with obstructive sleep apnea syndrome treated by nasal continuous positive airway pressure. Study of compliance and time-course of Apnea-Hypopnea index

Between 1990 and 1995, 369 patients were investigated for obstructive sleep apnea syndome (OSAS) by polysomnography. Among them, 248 patients with a mean Apnea-Hyponea index (AHI) of 37.7 per hour were treated by nasal continuous positive airway pressure (n-CPAP). Mean follow up was 39.5 ± 20.4 months. In this group, 23 patients (9.2 %) refused nCPAP immediately or after the first night and 39 (15.7 %) gave up later. 15 patients (6 %) died during the period of the study. The cumulative compliance reached 70 % at 72 months. Non compliant patients usually gave up n-CPAP before the end of the first year. We compared the group of 150 patients always treated at the date of 31/12/95 with the group of 62 patients who refused nCPAP initially or gave up later. There was no difference in clinical parameters or polysomnographic data between the two groups.

In 94 patients treated by nCPAP for more than a year we evaluated the outcome of AHI by a polysomnography performed after 72 hours of nCPAP cessation. Mean AHI of the group at this time was 38.2 ± 20.3/h and was well correlated with the initial index (r = 0.41, p < 0.0001). However for 28 patients (29.7 %) we observed, at the time of this second AHI determination, a variation (plus or minus) of at least 50 % of the index. 6 patients, without any significative weigth loss, had an AHI below 5/h at this second determination. In this small group nCPAP was interrupted for 6 to 12 months, then another polysomnography was performed. At this time mean AHI was 42.4/h and clinical symptoms had reappeared in all patients.

This study demonstrated that compliance to nCPAP in OSAS patients is good. No clinical or polysomnographic factors allow to predict non compliance. AHI is not modified by long term treatment with nCPAP.

Entre 1990 et 1995, nous avons recherché un syndrome d'apnée du sommeil (S.A.S.) par une polysomnographie (E.P.S.) et un bilan fonctionnel respiratoire chez 369 patients. Parmi eux, 248 patients ayant un index d'apnée-hypopnée ou I.A.H. de 37,7/heure en moyenne ont été appareillés par pression positive continue (P.P.C.). La durée moyenne de suivi était de 39,5 ± 20,4 mois. Dans ce groupe, nous avons observé à la date du 31/12/1995 : 15 décès (6 %), 23 refus immédiats (9,3 %), 39 abandons secondaires entre 1 et 12 mois (15,7 %) et 20 patients désappareillés (8 %) pour refus de prise en charge par la CPAM. L'adhérence à la P.P.C. a été évaluée par calcul actuariel à 70 % à 72 mois. L'essentiel des abandons survient entre le 3 e et le 12 e mois. L'analyse statistique a comparé le groupe des 150 patients toujours appareillés au 31/12/1995 et considéré comme observant, au groupe des 62 patients ayant refusé initialement la P.P.C. ou l'ayant secondairement abandonné. Aucun facteur clinique, fonctionnel respiratoire, oxymétrique ou polysomnographique n'était prédictif de l'observance de la P.P.C..

Nous avons ensuite pu déterminer chez 94 patients atteints de S.A.S. et traités par P.P.C. depuis plus d'un an, le devenir de l'I.A.H lors d'un nouvel E.P.S. réalisé 48 à 72 heures après arrêt de la P.P.C.. Lors du nouvel E.P.S., l'I.A.H. moyen était de 38,2/heure (± 20,3). Pour l'ensemble du groupe, il existait une bonne corrélation avec les résultats de l'E.P.S. initial (r = 0,41 ; p < 0,0001). Cependant pour 28 patients (29,7 %), on observait lors du 2 e E.P.S. une variation de 50 % (en plus ou en moins) de l'I.A.H. Chez 6 patients (I.A.H.moyen initial de 24,6/heure), en dehors de toute variation pondérale, l'I.A.H. moyen était de 4,2/heure lors du nouvel E.P.S.. Ces 6 patients ont été désappareillés pendant 6 à 12 mois. Un nouvel enregistrement a été réalisé, l'I.A.H. était alors en moyenne de 42,4/heure et dans tous les cas, la somnolence diurne qui avait disparu sous P.P.C. était réapparue.

En conclusion, cette étude montre que l'adhérence à la P.P.C est globalement bonne chez les patients présentant un S.A.S.. Il n'existe aucun facteur prédictif de l'observance de la P.P.C. chez ces patients. l'I.A.H. ne présente pas de modification au cours du temps chez la majorité des patients atteints de S.A.S. traités par P.P.C. Toutefois, le délai de 48 à 72 heures d'arrêt de la P.P.C. n'est pas toujours suffisant pour voir réapparaître les signes cliniques et polysomnographiques du S.A.S., en particulier chez les patients appareillés avec un I.A.H. initial inférieur à 30/heure.
