Introduction

La granulomatose lymphomatoïde est une maladie rare, grave, actuellement considérée comme un lymphome B à grandes cellules fréquemment associé au virus Epstein-Barr (EBV). Son traitement, mal défini, se résume à une corticothérapie isolée ou associée à l'endoxan ou à une polychimiothérapie.

Observation

Nous rapportons l'observation d'un patient asymptomatique adressé pour la découverte fortuite d'opacités nodulaires bilatérales. L'examen clinique et l'endoscopie bronchique étaient normaux. Le diagnostic de granulomatose lymphomatoïde a été retenu par examen anatomopathologique d'une biopsie pulmonaire chirurgicale. L'étude immunohistochimique a confirmé le phénotype B de cette maladie avec l'identification de grandes cellules atypiques CD 20+ et l'hybridation in situ, son association avec EBV.

Dans notre observation, il s'agissait d'une forme d'évolution lente. Le traitement par anticorps monoclonal anti-CD 20 (rituximab) a permis initialement une diminution des images parenchymateuses et des adénopathies médiastinales.

Conclusion

Ce traitement, récemment utilisé dans des formes pulmonaires de granulomatose lymphomatoïde, donne des résultats encourageants. Comme dans le traitement standard des lymphomes B agressifs, le Rituximab peut être associé à une polychimiothérapie.

Efficacy of Rituximab in Lymphomatoid Granulomatosis

Introduction

Lymphomatoid Granulomatosis is a rare and serious disease, now considered to be a B-cell lymphoma, which is frequently associated with Epstein-Barr virus infection. There is no consensus on treatment, which is usually based on steroid therapy, either alone or combined with cyclophosphamide and combination chemotherapy.

Case Report

We report the case of an asymptomatic patient diagnosed after the incidental discovery of bilateral nodular opacities on their chest x-ray. Physical examination and bronchoscopy were normal. The diagnosis of Lymphomatoid Granulomatosis was made on the basis of surgical lung biopsy. Immunohistochemical studies confirmed the B phenotype of the lymphoma with the identification of atypical large CD 20 positive cells. In situ hybridisation confirmed the presence of EBV. In this case the course of the disease was slow. Treatment with anti CD 20 monoclonal antibodies (rituximab) led initially to a reduction in parenchymal abnormalities and mediastinal adenopathy.

Conclusion

This treatment, recently used in Lymphomatoid Granulomatosis with pulmonary involvement, has shown promising results. Rituximab can be used in combination chemotherapy as standard treatment for aggressive B-cell lymphoma.
