Le pronostic des cancers bronchiques non à petites cellules (CBNPC) localement avancés non résécables reste mauvais, malgré les progrès thérapeutiques accomplis ces dernières années. La meilleure stratégie thérapeutique actuelle repose sur l’association concomitante de la chimiothérapie et de la radiothérapie. Ce traitement doit être réservé à des patients en bon état général. L’œsophagite en représente la toxicité limitante. La chimiothérapie doit être si possible administrée à pleine dose pendant la radiothérapie afin de contrôler la maladie micrométastatique. L’association cisplatine-étoposide reste un standard. L’association cisplatine-vinorelbine offre un assez bon rapport efficacité/tolérance. Le choix entre une chimiothérapie d’induction et une chimiothérapie de consolidation reste à définir. L’adjonction du docétaxel en consolidation après radio-chimiothérapie semble améliorer la survie, mais ces résultats nécessitent d’être confirmés par un essai randomisé de phase III. Les progrès technologiques récents permettent d’optimiser la radiothérapie, d’améliorer les résultats et de réduire les toxicités. La place des thérapeutiques ciblées dans la stratégie de prise en charge des CBNPC de stade III est en cours dévaluation.

Stage IIIB non-small cell lung cancer.

Is there a recommended treatment?

The prognosis of locally advanced inoperable non-small cell lung cancer (NSCLC) remains poor despite the advances in treatment achieved in recent years. Currently the best therapeutic strategy relies on concurrent radiotherapy and chemotherapy. This treatment should be reserved for patients of good performance status. Oesophagitis represents the limiting toxicity. If possible chemotherapy should be administered in full dosage during radiotherapy with the aim of controlling micrometastases. Combined cisplatin-etoposide remains a standard regime and cisplatin-vinorelbine offers a good efficacy/toxicity ratio. The choice between induction chemotherapy and consolidation chemotherapy remains uncertain. The addition of docetaxel as consolidation after radio-chemotherapy seems to improve survival but the results need to be confirmed by a randomised phase III trial. Recent technological advances in radiotherapy allow optimisation of the dose, improved results and reduced toxicity. The place of targeted therapies in the management strategy of stage III NSCLC is under investigation.
