Résumé

La chirurgie est le traitement de référence des CNPC de stades I, II et IIIA. La chimiothérapie adjuvante à base de sels de platine (ou néoadjuvante) permet un bénéfice de survie globale évalué à 5 % à 5 ans dans les stades II et IIIA et est recommandé. La radiothérapie adjuvante n’est pas recommandée dans les tumeurs N2. L’impact des traitements ciblés péri-opératoires des patients ayant des altérations moléculaires est actuellement suggéré par l’étude ADAURA chez des mutés EGFR démontrant un bénéfice de survie sans récidive. La place des inhibiteurs de points de contrôle de l’immunité est actuellement étudiée dans des essais néoadjuvants et adjuvants et les premiers résultats sont très encourageants. Les premiers résultats de l’essai de phase 3 CM-816, testant la combinaison nivolumab chimiothérapie versus chimiothérapie ont montré une supériorité de la combinaison, en termes de réponse pathologique complète et de survie sans évènement. L’essai de phase 3 comparant l’atezolizumab versus contrôle en adjuvant est positif, en termes de survie sans récidive, mais uniquement pour les stades II et III PD-L1 positif. Nous aborderons dans cette revue, les résultats des essais des inhibiteurs de points de contrôle immunitaires (ICI) avec ou sans chimiothérapie uniquement.

Abstract

Surgery is the standard of care for stage I, II, and IIIA NSCLC. Adjuvant platinum-based chemotherapy (or neoadjuvant) provides an estimated 5% overall survival benefitat 5 years in stages II and IIIA and is recommended. Adjuvant radiotherapy is not recommended for N2 tumors. The impact of peri-operative targeted therapies in patients with molecular alterations is currently suggested by the ADAURA study in EGFR mutated patients showing a benefit in terms of recurrence-free survival. The role of immune checkpoint inhibitors is currently being studied in neoadjuvant and adjuvant trials and the first results are very encouraging. Initial results from the phase 3 CM-816 trial, testing the combination of nivolumab and chemotherapy versus chemotherapy, showed superiority of the combination in terms of complete pathological response and event-free survival. The phase 3 trial comparing atezolizumab versus control in the adjuvant setting was positive in terms of relapse-free survival, but only for PD-L1 positive stage II and III. In this review, we will discuss the results of immune checkpoint inhibitor (ICI) trials with or without chemotherapy only.
