Les nouveautés en immunothérapie : nouvelles cibles, rationnel et données précoces dans les cancers broncho-pulmonaires non à petites cellules - 16/10/22

Résumé

Les stratégies thérapeutiques utilisant des anticorps anti-PD-(L)1 sont maintenant bien établies chez les patients avec un cancer du poumon non à petites cellules (CBPNPC) sans addiction oncogénique. Le bénéfice de l’immunothérapie, conduisant parfois à la guérison, reste limité aux tumeurs « chaudes » caractérisées par un microenvironnement tumoral inflammatoire. De nouvelles thérapeutiques doivent être développées pour potentialiser la cytotoxicité des lymphocytes T. Les anticorps ciblant les récepteurs de co-stimulation ou de co-inhibition sont évalués en association avec des anticorps anti-PD-(L)1. D’autres approches incluent les vaccins anticancéreux ou les virus oncolytiques. Globalement, l’objectif est de rediriger efficacement le système immunitaire contre la tumeur. Ceci pourrait être aussi atteint et optimisé par l’identification de biomarqueurs prédictifs spécifiques de la tumeur et du patient pour proposer de nouvelles indications thérapeutiques individualisées.

Abstract

Therapeutic strategies with anti-PD-(L)1 antibodies are now well established in patients with nonsmall-cell lung cancer (NSCLC) without oncogenic addiction. The benefit of immunotherapy, sometimes leading to healing, remains restricted to “hot” tumors characterized by an inflammatory tumor micro-environment. Consequently, there is a need to develop new therapeutics improving the cytotoxic T cell activity. Antibodies targeting co-stimulatory or co-inhibitory receptors are evaluated in combination with anti-PD-(L)1 antibodies. Other approaches include anti-cancer vaccines or oncolytic virus. Overall, the goal is to effectively redirected the immune system against the tumor. This could be achieved and optimized by the identification of predictive biomarkers specific to the tumor and the patient for new individual therapeutic proposals.
