Parmi les cancers bronchiques non à petites cellules oligométastatiques, deux situations peuvent être individualisées : l’oligoprogression et l’oligopersistance. Elles peuvent toutes deux survenir en cours de traitement systémique, mais elles se distinguent par la qualité du contrôle de la maladie néoplasique. En cas d’oligoprogression, il existe une reprise évolutive d’un nombre limité de sites tumoraux après une réponse initiale au traitement systémique. En revanche, en situation d’oligopersistance, la maladie néoplasique reste globalement contrôlée sous l’effet du traitement systémique, mais il persiste un nombre limité de sites tumoraux actifs. L’objectif de cet article est de synthétiser les données disponibles dans la littérature concernant l’utilisation des traitements localement ablatifs dans la prise en charge thérapeutique des cancers bronchiques non à petites cellules oligoprogressifs et oligopersistants.

Among oligometastatic non-small cell lung cancers, two situations can be identified: oligoprogression and oligopersistence. They can both occur during systemic treatment, but they differ in the quality of neoplastic disease control. In the case of oligoprogression, there is a progression of a limited number of tumor sites after an initial response to systemic treatment. On the other hand, in a situation of oligopersistence, the neoplastic disease remains globally controlled under the effects of systemic treatment, but a limited number of active tumor sites persist. The objective of this article is to synthesize the data available in the literature concerning the use of locally ablative treatments in the therapeutic management of oligoprogressive and oligopersistent nonsmall cell lung cancer.
