La chirurgie est le traitement de référence des CBNPC de stades I à IIIA. Plusieurs essais randomisés de phase III de chimiothérapie adjuvante ou néoadjuvante avec une chimiothérapie à base de platine ont été menés et ont démontré un gain de survie significatif évalué à 5 % à 5 ans dans les stades II et IIIA. Ce bénéfice a été confirmé par deux méta-analyses sur données individuelles. En revanche pour les stades I, le bénéfice de la chimiothérapie n’est que suggéré pour les stades IB > 4 cm. L’essai Lung-ART/IFCT-0503 a récemment montré l’absence de place de la radiothérapie adjuvante dans les tumeurs réséquées N2. L’immunothérapie prend sa place dans les stades perioperatoires. Deux essais de phase 3 ont montré un bénéfice en adjuvant mais n’ont pas eu d’approbation en europe sur les données actuelles. Quatre essais de phase III Checkmate-816, Neotorch, AEGEAN et KEYNOTE-671 ont montré un bénéfice de l’association d’un inhibiteur de points de contrôle immunitaire à la chimiothérapie en situation néoadjuvante avec une autorisation européenne basée sur les résultats de Checkmate-816 chez les patients ayant un PD-L1 ≥ 1 %.

Surgery is the standard treatment for stage I to IIIA NSCLC. Several randomized phase III trials of adjuvant or neoadjuvant chemotherapy with platinum-based chemotherapy have been carried out, demonstrating a significant survival gain estimated at 5 % at 5 years in stages II and IIIA. This benefit has been confirmed by two meta-analyses based on individual data. In stage I, however, the benefit of chemotherapy has only been suggested for stages IB > 4 cm. The Lung-ART/IFCT-0503 trial recently showed that adjuvant radiotherapy has no place in N2 resected tumors. Immunotherapy is taking its place in the perioperative stages. Two Phase 3 trials have shown adjuvant benefit, but have not been approved in Europe on the basis of current data. Four phase III trials Checkmate-81, Neotorch, AEGEAN and KEYNOTE-671 have shown significant benefit from combining an immune checkpoint inhibitor with chemotherapy in the neoadjuvant setting, with European approval based on the results of Checkmate-816 in patients with PD-L1 ≥ 1 %.
