Le traitement standard des carcinomes bronchiques non à petites cellules de stade III non résécables repose sur l’association d’une chimiothérapie et d’une radiothérapie concomitantes. Il n’existe pas d’association de chimiothérapie standard, et plusieurs bithérapies à base de cisplatine peuvent être utilisées. Les chimiothérapies d’induction et de consolidation n’apportent pas de bénéfice. Même si l’utilisation du géfitinib en maintenance après chimio-radiothérapie concomitante n’a pas d’intérêt, il existe un rationnel pour l’utilisation des thérapeutiques ciblées en association avec la radiothérapie. D’autres stratégies thérapeutiques intégrant thérapeutiques ciblées et radiothérapie méritent d’être testées.

Stage III NSCLC The role of the chemotherapist

Standard treatment for non resectable stage III non-small cell lung cancer consists of concurrent chemotherapy and thoracic radiotherapy. There is no standard chemotherapy regimen, and several cisplatin-based doublets can be used. Induction and consolidation chemotherapy have been shown to yield no survival benefit. Although maintenance gefitinib has no interest after concurrent chemoradiotherapy, there is a rational for the delivery of targeted therapies in combination with radiotherapy. Other strategies integrating targeted treatments and radiotherapy warrant investigations.
