Pneumonie nécrosante à Staphylococcus aureus secréteur de la toxine de panton valentine - 21/10/08

Introduction Staphylococcus aureus secréteur de la leucocidine de Panton Valentine (LPV) est une entité récemment décrite comme étant responsable de pneumopathie nécrosante, grevée d’une mortalité importante (75 %).

Observation Nous rapportons un cas de pneumonie à SAMS secréteur de LPV, compliquée de multiples lésions nécrosantes et d’une dystrophie bulleuse majeure, avec une récupération clinique satisfaisante, survenant chez un adulte jeune sans ATCD. Quelques jours après une infection virale, survenait un tableau de détresse respiratoire aiguë compliqué d’un état de choc septique et d’un SDRA. L’aspect atypique endoscopique, constitué de sécrétions bronchiques purulo-nécrotiques desquammantes, l’évolution défavorable malgré une antibiothérapie large spectre et la mise en évidence du SAMS PLV+ sur une hémoculture et le piège bronchique permettait de poser le diagnostic. L’évolution était marquée par la survenue de nombreux épisodes de pneumothorax avec de multiples lésions kystiques bilatérales scannographique. L’issue était finalement lentement favorable sous antibiothérapie (oxacilline, clindamycine) et corticothérapie.

Conclusion La pneumonie à SAMS PLV+ doit être évoquée chez le sujet jeune, sans ATCD en cas d’échec d’une antibiothérapie adaptée. La difficulté de prise en charge est liée à l’importance des lésions nécrotiques et à la nécessité d’employer des antibiotiques actifs sur le germe ainsi que sa leucotoxine.

Necrotizing pneumonia due to Staphylococcus aureus producing Panton-Valentine toxin

Background Necrotizing pneumonia caused by Staphylococcus aureus producing Panton Valentine Leukocidine (PVL) has been described recently and is associated with a high mortality (75%).

Observation We report a case of pneumonia due to PV secreting Staphylococcus aureus in a healthy young adult, complicated by multiple necrotizing lung lesions and major cystic changes, with a favourable final outcome. Acute respiratory failure with haemodynamic failure and ARDS developed a few days after an influenza-like illness. The appearances at fibreoptic bronchoscopy were atypical, consisting of a purulent, necrotic tracheo- bronchitis with desquamation. The initial unfavourable progress despite broad spectrum antibiotic therapy and, finally, the identification of PV leukotoxin in blood cultures and tracheal aspirate, led to the diagnosis. During the clinical course there were repeated pneumothoraces (8 drains) due to multiple bilateral cystic lesions. Ultimately progress was favourable following treatment with antibiotics (flucloxacillin and clindamycin) and steroids.

Conclusion It is important to consider pulmonary complications of the Panton-Valentine leukotoxin in a healthy young adult who presents with necrotizing pneumonia and an unfavourable outcome in spite of broad spectrum antibiotics. Treatment is difficult on account of the extent of the necrotizing lesions and the need to use antibiotics effective against both the bacterium and its toxin.
