Résumé

Introduction

L’hémangioendothéliome épithélioïde pulmonaire (HEP) est une tumeur rare à prédominance féminine d’origine vasculaire et de malignité intermédiaire.

Cas cliniques

Nous rapportons trois observations d’HEP. Si les trois diagnostics ont été confirmés sur des prélèvements chirurgicaux, il faut noter que dans un cas une biopsie trachéale sous fibroscopie souple a été contributive. Deux patients ont bénéficié d’une TEP au fluoro-desoxy-glucose (FDG) qui a montré une intense fixation des lésions. Ces trois présentations témoignent du caractère évolutif pléiomorphe de cette pathologie. En effet, dans le premier cas l’évolution est multifocale nécessitant plusieurs résections pulmonaires, dans le deuxième il n’a pas été noté de récidive après la chirurgie et chez le troisième patient le décès est rapidement survenu au décours d’un hémothorax.

Conclusion

L’HEP est habituellement découvert de façon fortuite. Son diagnostic est posé après étude immuno-histochimique de biopsies pulmonaires. Aucun standard thérapeutique n’est validé, l’abstention thérapeutique reste de mise dans les formes asymptomatiques. Dans les formes pauci- ou uninodulaires le traitement de choix est chirurgical. Le pronostic est variable. En général, l’évolution est lente sauf pour les formes avec hémoptysie, épanchement pleural, hémothorax, adénopathies médiastinales et localisations hépatiques.

Summary

Introduction

Pulmonary epithelioid haemangioendothelioma (PEH) is a rare vascular tumour of intermediate malignancy that predominantly affects women.

Clinical case reports

We report three cases of PEH. Though all three diagnoses were confirmed by surgical biopsy, it is notable that, in one case, a tracheal biopsy by flexible bronchoscopy was contributory. Two patients had undergone positron emission tomography, which showed the lesions to be well established. The three cases show that the evolution of this pathology can be very varied. In the first case, the disease was multifocal and needed to be treated with several pulmonary resections. In the second case no recurrence has been observed after surgery; in the third case the patient died following a haemothorax.

Conclusion

The discovery of a PEH is usually fortuitous. The diagnosis is confirmed by immuno-chemical analysis of a lung biopsy. No standard treatment has been validated and no treatment is necessary if the disease is asymptomatic. If there is only one nodule, surgery is the treatment of choice. The prognosis is variable. Usually the evolution is slow except when the disease is complicated by haemoptysis, pleural effusion, haemothorax, mediastinal lymphadenopathy or hepatic spread.
