Résumé

Introduction

La néovascularisation est une des caractéristiques de la maladie cancéreuse. Le Vascular Endothelial Growth Factor (VEGF) est directement impliqué dans la régulation de cette néoangiogénèse tumorale, notamment dans le cancer bronchique. Le bévacizumab est un anticorps monoclonal humanisé ciblant le VEGF couramment utilisé en oncologie thoracique. Parmi ces effets indésirables, la perforation de la cloison nasale reste rare.

Observation

Nous rapportons le cas d’une patiente de 50ans, suivie pour un adénocarcinome bronchique lobaire supérieur droit, d’emblée métastatique, traité par cinq cures de chimiothérapie (cisplatine, gemcitabine, bévacizumab), ayant présenté une perforation de la cloison nasale, après sa troisième injection de bévacizumab en monothérapie, durant la phase de maintenance.

Conclusion

Il s’agit, à notre connaissance de la première description de cette complication au cours du traitement d’un carcinome bronchique. Sa prise en charge est purement symptomatique. La conduite à tenir vis-à-vis du bevacizumab n’est pas du tout codifiée.

Summary

Introduction

Neovascularisation is a hallmark of cancer. Vascular endothelial growth factor (VEGF) is directly involved in the regulation of this tumoural neoangiogenesis, especially in lung cancer. Bevacizumab is a humanized monoclonal antibody targeting VEGF that is commonly used in thoracic oncology. Among the side effects, perforation of the nasal septum is rare.

Case report

We report the case of a 50-year-old woman with adenocarcinoma of the right upper lobe, immediately metastatic, treated with five cycles of chemotherapy (cisplatin, gemcitabine, bevacizumab). She developed a perforation of the nasal septum after the third injection of bevacizumab given as monotherapy during the maintenance phase.

Conclusions

This is, to our knowledge, the first description of this complication in the course of treatment of bronchial carcinoma. Its management is purely symptomatic. The action to be taken in using bevacizumab is not fully codified.
