Radiothérapie en conditions stéréotaxiques des patients inopérables - 09/01/16

La radiothérapie en conditions stéréotaxiques (ou SBRT : Stereotactic Body Radiation Therapy) est un changement fondamental par rapport à la radiothérapie classique fractionnée et représente une nouvelle indication thérapeutique. Elle est maintenant un traitement reconnu pour les patients inopérables ou qui refusent la chirurgie. Les résultats sont encourageants avec des taux de contrôle local (80 à 100 % à 2 ans), et de survie parfois très élevés (56 à 80 % à 2 ans) dans des populations sélectionnées. Les taux de toxicité tardive restent acceptables autour de 10 à 20 % de grade ≥ 3 à 2 ans. Sa bonne tolérance la rend indiquée mâme pour des patients âgés et fragiles. Comparativement à une irradiation classique, la radiothérapie en conditions stéréotaxiques entraîne des modifications radiologiques différentes. Il est nécessaire de standardiser les critères d’évaluation cliniques, radiologiques et fonctionnels afin de mieux différencier une éventuelle rechute d’une pneumopathie interstitielle radio-induite.

Stereotactic radiotherapy is a fundamental change from the conventional fractionated radiotherapy and represents a new therapeutic indication. Stereotactic radiotherapy is now a standard of care for inoperable patients or patients who refuse surgery. The results are encouraging with local control (80 to 100% at 2 years) and survival (56 to 80% at 2 years) rates very high in selected populations. The rate of late toxicity remains acceptable around 20% of grade ≥ 3 at 2 years. Good tolerability makes it appropriate even for elderly and frail patients. Compared to conventional radiation, stereotactic radiotherapy results in different radiological changes. It is necessary to standardize the clinical, radiological and functional endpoints to better differentiate a relapse from radiation-induced interstitial lung disease.
