Introduction

L'apparition d'une adénopathie périphérique en fin d'un traitement antituberculeux d'une tuberculose pulmonaire est un phénomène rare dont la prise en charge, particulièrement thérapeutique, ne figure pas sur les recommandations de l'OMS.

Cas clinique

Il s'agit du cas d'une patiente âgée de 36 ans ayant présenté, en fin du traitement antituberculeux pour une tuberculose pulmonaire infiltrative et cavitaire diffuse, une adénopathie axillaire gauche de taille importante.

Le traitement a consisté en une ponction aspiration à l'aiguille fine ainsi que la prolongation de la durée du traitement antituberculeux à 9 mois.

L'évolution était favorable sur le plan radioclinique et bactériologique.

Conclusion

La ponction aspiration d'une adénopathie paradoxale en fin du traitement antituberculeux ainsi que la prolongation de la durée du traitement semblent efficaces et pourraient constituer une alternative à l'excision chirurgicale.

Paradoxical lymphadenopathy following treatment of pulmonary tuberculosis. A rare condition with controversial management

Introduction

The appearance of a peripheral lymph node tuberculosis in end of anti-tuberculosis treatment of a pulmonary tuberculosis is a rare phenomenon whose assumption of responsibility, particularly therapeutic, is not reproduced on the recommendations of WHO.

Clinical case

It is about a case of a 36 years old patient who presented, in end anti-TB treatment for ulcéroinfiltrative pulmonary tuberculosis, a left axillary adenopathy of important size.

The treatment consisted with a puncture aspiration with the fine needle as well as the prolongation of the duration of the anti-TB treatment from 6 to 9 months.

The evolution was favourable on radio-clinical and bacteriological level.

Conclusion

The puncture-aspiration of a paradoxical adenopathy in end of the anti-TB treatment as well as the prolongation of the duration of the treatment is effective and could constitute an alternative to the surgical excision.
