Introduction |
La capnographie transcutanée (P tc CO 2 ) permet une estimation non invasive de la pression partielle artérielle en dioxyde de carbone et constitue un outil de plus en plus utilisé pour l’évaluation de l’hypoventilation alvéolaire, notamment nocturne, chez les patients atteints d’insuffisance respiratoire chronique. Son utilisation clinique nécessite toutefois une bonne maîtrise de ses principes techniques, de ses conditions de réalisation et de ses modalités d’analyse afin d’en garantir la fiabilité.
Méthodes |
Ce travail repose sur des avis d’experts du groupe « Assistance ventilatoire et oxygénothérapie » (GAVO 2 ) de la Société de pneumologie de langue française (SPLF). Les données disponibles dans la littérature ont été analysées et discutées afin de proposer des conseils pratiques concernant la réalisation, l’analyse et l’interprétation des enregistrements nocturnes de P tc CO 2 chez l’adulte.
Résultats |
Dans des conditions techniques optimales, la P tc CO 2 est bien corrélée à la PaCO 2 , avec un biais généralement inférieur à 0,5 kPa. La qualité de l’enregistrement est un élément déterminant et repose sur une pose adaptée du capteur, un enregistrement continu de durée suffisante et une analyse préalable des courbes afin d’identifier les artéfacts et les dérives de mesure. Les définitions de l’hypoventilation nocturne basées sur la P tc CO 2 restent hétérogènes et reposent essentiellement sur des avis d’experts.
Conclusion |
La capnographie transcutanée constitue un outil non invasif pertinent pour l’évaluation de l’hypoventilation alvéolaire chez l’adulte atteint d’insuffisance respiratoire chronique, à condition d’en maîtriser rigoureusement les aspects techniques et interprétatifs. Elle doit être intégrée en complément des gaz du sang dans une approche clinique globale.

Introduction |
Transcutaneous capnography (P tc CO 2 ) provides a non-invasive estimate of partial arterial pressure of carbon dioxide in the blood and is an increasingly widely used tool for assessing alveolar hypoventilation, particularly nocturnal hypoventilation, in patients with chronic respiratory failure. That much said, its reliability in clinical practice requires satisfactory understanding of the relevant technical principles, conditions of implementation, and analysis methods.
Methods of analysis |
This work is based on expert opinions from the Ventilatory Assistance and Oxygen Therapy Group (GAVO 2 ) of the French-speaking Pulmonology Society (SPLF). The data available in the literature were analyzed and discussed in view of providing practical advice on the performance, analysis, and interpretation of nocturnal P tc CO 2 recordings in adults.
Results |
Under optimal technical conditions, P tc CO 2 correlates well with PaCO 2 , with bias generally lower than 0.5 kPa. The quality of recording is a determining factor and depends on proper sensor placement, continuous recording for a sufficient amount of time, and prior analysis of the curves aimed at identifying artifacts and measurement drifts. Definitions of nocturnal hypoventilation based on P tc CO 2 remain heterogeneous and are based primarily on expert opinion.
Conclusion |
Transcutaneous capnography is a relevant non-invasive tool ensuring assessment of alveolar hypoventilation in adults with chronic respiratory failure, provided that the technical and interpretative aspects are fully mastered and rigorously applied. It should complement blood gases as part and parcel of a comprehensive clinical approach.
