Competing interests: Salim Yusuf holds a Heart and Stroke Foundation Chair for Cardiovascular Research; he is the executive director of the Population Health Research Institute and the vice-president of research at Hamilton Health Sciences; he was a senior scientist at the Canadian Institutes for Health Research (CIHR), from whom he has received funding; he holds a Heart and Stroke Foundation of Ontario research chair and has received peer review funding in the United Kingdom, the United States, other countries and from several companies. John Cairns has received financial support for his research from the Medical Research Council of Canada, CIHR and the Heart and Stroke Foundation of Ontario; he has received payment for lectures from Boerhinger Ingelheim; he has been reimbursed for travel expenses by Sanofi and Bristol Meyers Squibb; he was the project leader of the CIHR Clinical Research Initiative from 2004 to 2005.
