L’impact de la chimiothérapie en terme de survie par rapport aux soins palliatifs seuls dans les cancers bronchiques non à petites cellules (CBNPC) a été confirmé par plusieurs méta-analyses. En première ligne, le développement des drogues récentes (vinorelbine, gemcitabine, paclitaxel, docétaxel) douées d’un meilleur ratio efficacité-toxicité a conduit à individualiser comme standard thérapeutique chez les patients en bon état général 4 à 6 cycles d’une bithérapie associant un sel de platine et d’une drogue récente active, avec une survie à 1 an voisine de 35 %.

Le choix du traitement est basé sur les différences de profils de toxicité entre les schémas thérapeutiques tenant compte des co-morbidités. Les patients fragilisés (patients âgés ou avec altération de l’état général) bénéficieront d’une monothérapie active ; l’intérêt d’une bithérapie dépourvue de cisplatine est possible.

Les cytotoxiques récents semblent avoir apporté un progrès réel, quoique d’amplitude modeste dans le traitement des CBNPC métastatiques, progrès surtout tangible si l’on considère la proportion des patients vivant à plus long terme.

Chemotherapy for metastatic non-small cell lung cancer

Treatment of stage IV non-small cell lung cancer (NSCLC) is based upon chemotherapy, with several meta-analyses showing an improvement of survival compared to supportive care alone.

For first-line chemotherapy, the development of new third generation drugs (vinorelbine, gemcitabine, paclitaxel, docétaxel) with a better therapeutic index has led to the definition of 4 to 6 cycles of a platinum-based dual agent regime as standard therapy for performance stage (PS) 0-1 patients, producing a 1-year survival in the region of 35%.

The choice of agents is based upon the differences in the toxicity profile of platinum-based combinations and on patient co-morbidities. For frail patients (PS>2 and elderly patients), single-agent chemotherapy seems more appropriate than cisplatin-based combinations; carboplatin or non-platinum combinations might be more effective.

Third generation cytotoxic agents have modestly improved survival for patients with stage IV NSCLC but should probably provide an increased number of long-term survivors.
