Résumé

Le terme de fibro-élastose pleuro-parenchymateuse apparaît dans la littérature en 2004.

Le diagnostic est suspecté devant un patient dyspnéique avec une présentation radiologique associant un épaississement pleural et une fibrose sous pleurale très prédominante dans les lobes supérieurs avec diminution du volume pulmonaire, ascension des hiles, rétraction puis densification des sommets.

Le diagnostic de certitude est anatomopathologique par marquage des fibres d’élastine.

Les diagnostics différentiels sont les séquelles de tuberculose, certaines connectivites ou polyarthrites, l’asbestose et les « coiffe apicale ».

La maladie peut survenir à tout âge. La dyspnée est constante, la toux, l’amaigrissement et les douleurs thoraciques fréquentes. Le platythorax est caractéristique. La fonction respiratoire est altérée avec un syndrome restrictif majeur.

Les pneumothorax sont une complication récurrente. L’évolution se fait vers une insuffisance respiratoire restrictive le plus souvent hypercapnique.

Les étiologies sont principalement idiopathiques et souvent iatrogènes (transplantation pulmonaire, post chimiothérapie, greffe de moelle osseuse), mais il existe des formes familiales avec anomalies génétiques.

Le pronostic est sombre et la dégradation de la capacité vitale extrêmement rapide.

Les corticoïdes sont inefficaces. Il n’y a actuellement pas de traitement efficace connu hormis la transplantation pulmonaire.

Abstract

Idiopathic pleuroparechymal fibroelastosis appears in the litlerature under that name in 2004.

The diagnosis is usually suspected on the basis of a patient with dyspnea and a compatible radiographic pattern with a pleural thickening as well as a subpleural fibrosis in the uppers lobes and a decreasing pulmonary volume, an ascension of the pulmonary hiliums and a shrinking of the apex.

The definitive diagnosis requires anatomopathological analysis with specific colouring of Elastin fibres.

Differential diagnosis are tuberculosis consequences, connective tissue disease, polyarteritis, the lung asbestosis, and the apical cap.

The disease can appear at any age, dyspnea is still present, caughs, weight loss and chest pain are common. Platythorax is typical. Lung fonction is altered with a restrictive spirometry pattern. Spontaneous pneumothorax is a common complication. The disease progresses toward a restrictive and an hypercanic respiratory insufficiency.

Etiologies are idiopathic and commonly iatrogenic (lung transplantation, chemotherapy, bone marrow transplantation). There also are familial forms with genetics abnormalities.

The prognosis is poor with a fast vital capacity decreasing.

The corticosteroids are useless. At the moment there is no effective treatment other than lung transplantation.
