Principes et techniques de la radiothérapie thoracique des carcinomes bronchiques non à petites cellules (CBNPC) - 17/12/18

La radiothérapie thoracique est une arme thérapeutique indispensable à la prise en charge des formes localisées inopérables des carcinomes bronchiques non à petites cellules (CBNPC) qui s’articule avec la chirurgie et les traitements généraux, chimiothérapie, thérapies ciblées et immunothérapie, dans une démarche pluridisciplinaire. Au cours des vingt dernières années, elle a connu des avancées technologiques considérables grâce aux progrès de l’imagerie, de l’informatique et à la modernisation des accélérateurs, passant d’une irradiation en 2D à une technologie 4D en routine. Le présent article a pour but de décrire les principes et les différentes techniques de radiothérapie thoracique à disposition des cliniciens : radiothérapie conformationnelle 3D sans ou avec modulation d’intensité, gating respiratoire, radiothérapie adaptative et guidée par l’image et en conditions stéréotaxiques.

Thoracic radiotherapy is an essential therapeutic weapon for localized inoperable nonsmall cell lung cancer (NSCLC) which articulates with surgery and general treatments as chemotherapy, targeted therapies and immunotherapy, in a multidisciplinary approach. During the last twenty years, radiotherapy has brought major technological improvements thanks to progress in imaging, computing, LINAC design, moving out of 2D irradiation into a 4D technology for routine use. The purpose of this paper is to describe principles and the various techniques of thoracic radiotherapy available for the clinicians: 3D conformational radiotherapy without or with modulation of intensity, adaptative and image-guided radiotherapy, respiratory gating and stereotactic radiotherapy.
