Résumé

La dernière version de la classification internationale pour les cancers bronchiques, basée sur le TNM et la série de Mountain, date de 1997 et comporte de nombreuses imperfections. L’International Association for the Study of Lung Cancer a élaboré une étude rétrospective multicentrique internationale de grande taille (IASLC International Staging Project) qui a permis d’apporter des modifications majeures. L’étude a souligné, pour l’évaluation du T, le rôle de la taille de la tumeur primitive, avec des seuils à 2, 3, 5 et 7 cm, et des nodules satellites. Les épanchements malins sont versés dans les stades M1. La nouvelle classification est également applicable aux cancers bronchiques à petites cellules et aux tumeurs carcinoïdes. Une nouvelle cartographie ganglionnaire est proposée, précisant plus clairement les limites de chaque station. Les modifications apportées par l’IASLC Staging Project ont été acceptées par l’UICC pour la 7e édition de la classification internationale.

Summary

The previous version of the international classification for lung cancers, based on the TNM and the Mountain series, dated from 1997 and had numerous imperfections. The International Association for the Study of Lung Cancer has undertaken an international large scale retrospective multicentre study (IASLC International Staging Project) which has allowed substantial modifications. The study underlines, for the T descriptor, the role of tumour size, with thresholds at 2, 3, 5 and 7 cm, and of satellite nodules. Malignant effusions are now included in the M1 stage. The new classification is also applicable for small cell lung cancers and carcinoid tumours. A new nodal map is proposed, delineating more precisely the limits of each station. The modifications from the IASLC Staging Project have been accepted by the UICC for the 7th edition of the international classification.
